Post Discharge Care:

Having help at home in the immediate aftermath of a hospital visit can be critical.  Adherence to discharge care instructions regarding medications, follow-up visits, physical therapy or other instructions are essential for a successful and full recovery and to avoid being readmitted to the hospital. According to Centers for Medicare and Medicaid Services (CMS), one in five Medicare patients are readmitted to the hospital within 30 days of being discharged.  We can meet you in the hospital before being discharged so that a coordinated plan and care team are in place to minimize the risks of a re-hospitalization and to assist in the recovery process.

End of Life Care:

Hospice can be an extremely challenging and stressful experience.  Though hospice agencies have staff that make home visits by appointment or in case of an emergency, they generally do not provide continuous care to assist with the monitoring or on-going comfort of the hospice patient.  We have worked in tandem with the leading hospice agencies throughout Southern California to provide support and guidance to hospice patients and those close to them.  We have encountered many situations where people close to the hospice patient want to be there emotionally for them, but are unable to or prefer not to deal with the physical aspects of their care.
